Application
submitted:
Those who apply for compliance approval with regard to "sale use" TTE should submit to ETC by post, express or send by yourself.
for
new TTE:
Those who apply for new TTE compliance approval should complete application form and submit it together with copies of related identification and technical documents to ETC.
The
applicant who are foreign manufacture shall authorize the agent in Taiwan with the letter
of authorization (free form) to submit the application to ETC.
for
re-issue:
Those who have lost their TTE approval certificates issued by ETC may apply for re-issue to ETC with completing application form, adding "re-issue" and certificate No., and submit it with the declaration (free form) of lost .
for
replacement
Those who have damaged their TTE approval certificates issued by ETC may apply for replacement to ETC with completing application form and adding "replacement" and certificate No. on it.

series products
The application for compliance approval with regard to TTE should be submitted with definite brand name and model number. The products of the same series should be submitted in one application package.

Note: The application for compliance approval with regard to TTE should be submitted with definite brand name and model number. The products of the same series should be submitted in one application package.

Once
the approved TTE is modified in respect of model number, design or function, it
should be re-submitted for compliance approval. Nevertheless, after modification
in respect of design or function, if it remains in compliance with the relevant
technical specifications, the applicant only needs to notify the ETC the part of
modification for reference.
When the aforementioned TTE extends its telecommunications interfaces and does
not affect the function of the approved telecommunications interfaces, it is
permitted to conduct the compliance approval only upon the part of extension.
Those who apply for re-issue or replacement their TTE approval certificates, or
extending telecommunication interfaces should complete application form and
adding "re-issue", "replacement", "extension" and
original certificate No. on it.
Requirement
of Related Identification Documents
A copy of each related identification documents: including the front and rear sides, clearly noted “identical to the original”, affixed the seal/signature of the applicant and its representative; DGT or ETC may require the applicant to submit the original for review, if necessary
Domestic vendor
The applicant shall submit company license, business registration certificate or factory registration certificate
Foreign manufacturer
The applicant shall submit the certificate of establishment for its legal entity: example the business license or business/branch registration certificate.
The applicant shall authorize the agent in Taiwan with the letter of authorization (for reference) to submit the application to ETC.
Controlled telecommunications radio-frequency
devices
The importer shall submit the import permit issued by the Ministry of Transportation and Communication for controlled telecommunications radio-frequency devices and license of operation for controlled telecommunications radio-frequency devices.
The manufacturer shall submit license of operation for controlled telecommunications radio-frequency devices
Note: The "controlled telecommunications equipment" referred to Regulations On Inspection And Certification Of Controlled Telecommunications Equipment means those controlled frequency emitting equipment specified in a public notice (chinese only) published by the Ministry of Transportation and Communications in accordance with Section 2, Article 49 of the Telecommunications Act.

test report of telecommunication interface on

The equipment shall be tested following the specifications of the category and functions of certification. The test report shall contain the followings:
I. Equipment name, brand name and model number.
II. Testing facility connection diagram and description.
III. Name, brand name and model number of test instruments.
IV. Test items and standards.
V. Test data and verdict of test results.
VI. Date of acceptance and completion of testing.
If the recognized testing laboratory (RTL) is the accredited laboratory by Chinese National Laboratory Accreditation (CNLA), the test reports should be satisfied the requirements of ISO 17025; the scope of the accredited by CNLA should include the accredited mark of CNLA.
Other information specified by DGT of MOTC, R.O.C.
by the announcements of DGT and BSMI about TTE.

The equipments in accordance with the related announcements of DGT and BSMI shall submitted with the original EMI type test report conformed to CNS13438, limits and methods of measurement of radio interference characteristics of information technology equipment.
Original
test report and related data of safety
on the equipment sample
The equipments in accordance with the related announcements of DGT and BSMI shall submitted with the original safety type test report conformed to CNS14336, safety information technology equipment. The application must include:
(1)Product catalogue and a color photo (including the appearance and internal structure).
(2)Certification sheet of critical component or component data sheet.
(3)Circuit diagram, block diagram & PCB layout diagram
(4)Critical component list
(5)The original copy of the EMC type test report.
